The Company
Horizon Physical Therapy (“Horizon”) is a provider of outpatient physical therapy services with two locations in Georgia.

The Deal
Phoenix Physical Therapy (“Phoenix”), a portfolio company of Audax Private Equity (“Audax”), has acquired Horizon. This transaction represents Phoenix’ eighth acquisition since Audax’ initial investment in December 2018.

The Thesis
In December 2018, Audax partnered with Phoenix, a leading provider of outpatient physical therapy. This acquisition exemplifies Phoenix’s strategy of partnering with leading therapists in both new and existing markets. Phoenix plans to work with existing staff members to build on Horizon’s current market position and accelerate growth, while maintaining focus on patient outcomes and therapist satisfaction.
